   Could future editions please have a better index. Syntactic categories
   don't seem to appear in it at the moment (e.g. lhs). I suggest that
   all types, classes, defined functions and operators, syntactic categories,
   specials, reservedops and reservedids should be in it.

Yes, the indexing needs some work; not surprisingly it was only done
during the final phases of putting the report together.

   May I also suggest that there should be, in an appendix possibly, a table
   of the fixities and precedences of prelude-defined operators?

Good idea.
